I guess a large part of it is body language, too.
People can react to any number of things. In the example I described, she saw me realize who she was (I had already said something that identified me as a Trek fan). She reacted to the fact that I knew who she was but didn't say or do anything weird.
It's just that whenever I've had one of these encounters, it's been when I've been immediately identifiable as a concom member; whereas when I'm incognito (no headset, nondescript t-shirt), guests don't tend to react to me, Garrett Wang being the exception on Monday afternoon.
Some people would assume concom members would be safe.
